Can I view claim information online?
Yes! To view your group claims history and plan details visit the Group Web Platform and sign in or register using your CariCARE card/ policy number.
My child is 19-25 years old, and is enrolled full time as a student. How can I keep them active on the health plan?
Fill out the Annual Student Certification Form, and attach a letter from the University or School confirming full time enrollment in school, for EACH school year. Failure to provide these documents could result in removal from policy.
I lost my CariCare insurance card, what do I do?
Fill out the CariCare Card Replacement Request Form and provide $25 for the card.
